Position Summary

The Social Service Director oversees the planning, implementation, and evaluation of social service programs within a healthcare facility (such as a hospital, nursing home, or rehabilitation center). This role ensures that patients’ psychosocial, emotional, and practical needs are met in alignment with medical treatment goals and regulatory standards. The director also supervises social work staff and collaborates with interdisciplinary teams to support high-quality patient care.

Key Responsibilities

  • Program Leadership
  • Develop, implement, and evaluate social service policies and procedures.
  • Ensure compliance with federal, state, and local healthcare regulations.
  • Coordinate psychosocial assessments and discharge planning for patients.
  • Patient & Family Support
  • Advocate for patients’ rights and dignity.
  • Provide counseling, crisis intervention, and resource referrals to patients and families.
  • Address issues such as adjustment to illness, grief, financial hardship, and community reintegration.
  • Collaboration & Communication
  • Serve as a liaison between patients, families, medical teams, and community agencies.
  • Participate in interdisciplinary care conferences and contribute to care planning.
  • Build partnerships with external agencies for patient support services.
  • Team Management
  • Recruit, train, supervise, and evaluate social work staff.
  • Provide mentorship, professional development, and performance feedback.
  • Ensure adequate staffing coverage to meet patient needs.
  • Administrative Duties
  • Maintain accurate documentation in compliance with HIPAA and facility standards.
  • Manage departmental budget and resource allocation.
  • Prepare reports for senior leadership and regulatory agencies as required.

Qualifications

Education

  • Minimum of a Bachelor’s Degree in Social Work (BSW) or a Bachelor’s degree in an approved related field, including:
  • Sociology
  • Gerontology
  • Special Education
  • Rehabilitation Counseling
  • Psychology
